Telephone company service. Authorizes the Indiana utility regulatory commission to review a telephone company's provision of service to its customers in part or all of the telephone company's service area upon the commission's motion, at the request of the office of the utility consumer counselor, or upon the filing of a complaint by a customer of the telephone company. Requires the commission to conduct a review if a complaint is filed by a municipality any part of which is located within the telephone company's service area. Requires the commission to find severe deficiencies in provision of service to its customers if the commission finds (1) the telephone company has failed to provide service in that part of its service area under review that is comparable to service it provides to comparable communities in other parts of its service area; or (2) the telephone company has inadequately responded to recurring service problems identified to the telephone company by a significant number of its customers in the part of its service area under review. Authorizes the commission to order remedies, including imposition of civil penalties and revocation of the telephone company's authority to do business in all or part of its service area.
